Licensure - Wisconsin

This guide is to help you through the application process for a full medical license.

The licensure process should be started no later than February of the appointment year if possible.

Instructions for completing the application for medical license

It is the policy of Mayo Clinic School of Graduate Medical Education that all eligible trainees obtain and maintain a Wisconsin medical license. Start the licensure process for your appointment if you meet the requirements. Review the WI Board Eligibility Requirements. If you believe you do not meet the Wisconsin eligibility requirements, please contact Tisha Doherty.

Mayo Clinic reimburses licensure fees once you begin your training program and your license is issued. Keep a copy of your receipts, as you'll need these to be reimbursed. Mayo Clinic will not reimburse Federation Credentials Verification Service (FCVS) fees.

Exam deadlines

Mayo Clinic School of Graduate Medical Education (MCSGME) requires that your final licensing exams be taken as follows:

Trainees MCSGME deadline
Residents (PGY-1) U.S./Canadian graduates and international medical graduates By May 1 of the first MCSGME appointment training year*
Trainees who have completed or are completing the required ACGME** accredited training (or other graduate training as approved by state board requirements) at another institution Exam taken and score results received prior to start date
Fellows who have no prior ACGME accredited training (or other graduate training as approved by state board requirements), appointed to greater than two years of training in an accredited program By May 1 of the first MCSGME appointment training year*

*Off-cycle trainees to complete the requirement within the first ten months of training
