Application Process
Positions
The Nurse Practitioner or Physician Assistant Family Medicine Fellowship admits one trainee each year on a competitive basis.
Admission requirements
Prerequisites
To be eligible for this fellowship, you must meet the following requirements:
- Graduated from an accredited Physician Assistant or Advanced Practice Registered Nursing program
- National board certification
- Florida state license; obtain by program start date
- Current Drug Enforcement Administration (DEA) license is optional
- Current Basic Life Support certification (BLS required)
Note: If you have not yet graduated from your training program and not yet met all of the prerequisites, you may still apply for the fellowship. But to be successfully appointed to the fellowship, you must have graduated and met all of the prerequisites before the start of the program.
Candidates are selected based on grades and work experience, as well as volunteer experience, leadership qualities, and representation of personal qualities and skills.

Non-U.S. citizen applicants
Admission to Mayo Clinic School of Health Sciences is open to U.S. workers in the four categories below. Therefore, visa sponsorship is not available.

Applicants whose primary language is not English must submit results from the Test of English as a Foreign Language Internet-based test (TOEFL iBT). Scores from the speaking portion of the exam are given particular consideration in admission decisions. Learn more about the exam and register online at the Educational Testing Service.
The Mayo Clinic School of Health Sciences code for the TOEFL iBT is 5784.
Educational transcripts from schools outside the U.S. must be translated (if they are not already in English) and evaluated for U.S. equivalence by an accredited credential evaluation service company prior to submission. Please refer to the National Association of Credential Evaluation Services for a list of qualified companies. The applicant pays for the examination and credential-evaluation services.

Application instructions
Complete the following steps to apply:
- Create an account to begin the online Application for Admission.
- Select - Mayo Clinic School of Health Sciences
- Select - NP/PA Family Medicine Fellowship (FLA)
- Complete each section of the application and submit.
- After submission, view the required Supplemental Items and Documents
- Upload each required item in the Supplemental Items section
- Complete the Recommendation Request section
Additional required items
- Curriculum Vitae
- Three letters of recommendation are required and must be completed in the Recommendations Request section. Each recommender will receive an email with a link to complete a rating form and upload a letter.
- Personal statement which reflects professional goals and intent. This should describe your past professional experience and how the fellowship will further develop your career goals.
- Unofficial college transcripts may be uploaded in the Supplemental Items area for application purposes.
- Should you be accepted into the fellowship, the following documents are required prior to the start date.
- A copy of your current RN license and verification of license eligibility in the state of Florida, if applicable
- A copy of a current NP or PA license in the state of Florida
- Verification of board certification or eligibility as a Family Nurse Practitioner
- A copy of current Basic Life Support
- A copy of current Drug Enforcement Administration provider number if applicable
Interview process
The selection committee carefully reviews each application. Candidates selected for interviews will be notified by early May. Individuals considered for admission may complete a video, although an in-person interview is strongly encouraged to meet the faculty and tour the facilities. Interviews will be conducted May and June.
Acceptance
Acceptance letters are mailed approximately two weeks after interviews are completed. If you receive an acceptance letter, your reply is expected within 10 days.
Candidates accepting their admissions offers are then sent formal paperwork that initiates the final eligibility process, which includes a background check, drug screening, and verification of all information submitted during the application process.
