Positions
The Nurse Practitioner or Physician Assistant Endocrinology, Diabetes, and Metabolism Fellowship admits one trainee each year on a competitive basis at Mayo Clinic’s campus in Jacksonville, Florida.
Admission requirements
Prerequisites
To be eligible for the Endocrinology, Diabetes, and Metabolism Fellowship, you must meet the following requirements:
Nurse practitioner applicants:
- Graduate, or will graduate, from an accredited NP program
- National board certification as a family nurse practitioner (FNP) or adult-gerontology primary care nurse practitioner (AGPCNP) through AANC or AANP
- Licensed, or eligible to be licensed, as a nurse practitioner in the State of Florida
- Basic Life Support (BLS) certification
Physician assistant applicants:
- Graduate, or will graduate, from an accredited PA program
- Have a Physician Associate National Certifying Exam (PANCE) certification through NCCPA
- Licensed, or eligible to be licensed, as a physician assistant in the State of Florida
- Basic Life Support (BLS) certification
Note: If you have not graduated from your NPPA program, and currently do not meet all prerequisites, you may apply providing that prerequisites are expected to be met prior to the program start date.

Non-U.S. citizen applicants
Admission to Mayo Clinic School of Health Sciences is open to U.S. workers in the four categories below. Therefore, visa sponsorship is not available.

Applicants whose primary language is not English must submit results from the Test of English as a Foreign Language Internet-based test (TOEFL iBT). Scores from the speaking portion of the exam are given particular consideration in admission decisions. Learn more about the exam and register online at the Educational Testing Service.
The Mayo Clinic School of Health Sciences code for the TOEFL iBT is 5784.
Educational transcripts from schools outside the U.S. must be translated (if they are not already in English) and evaluated for U.S. equivalence by an accredited credential evaluation service company prior to submission. Please refer to the National Association of Credential Evaluation Services for a list of qualified companies. The applicant pays for the examination and credential-evaluation services.

Complete the following steps to apply:

- Complete each section of the application and submit.
- After submission, view the required Supplemental Items and Documents
- Upload each required item in the Supplemental Items section
- Complete the Recommendation Request section
Additional required items
- Recommendation form completed by two references (professor, faculty, supervisor, or preceptor)
- Personal statement (maximum 500 words) that includes your response to the following prompt:
- Describe your personal and professional experiences and how they relate to your interest in the program, your success in completing the program, and a career in endocrinology.
- Unofficial transcript. Note: If admitted, official copies of all transcripts will be required prior to the start of the program. They must be sent directly from the institution by email to mcnally.allyson@mayo.edu and peres.steven@mayo.edu, or by postal mail to:

Interview process
Candidates are selected based on grades and work experience, as well as volunteer experience, leadership qualities, and representation of personal qualities and skills. The selection committee carefully reviews each application. Individuals considered for acceptance are offered interviews. Interviews may be conducted virtually or in-person.
Acceptance
Acceptance notice will be sent by the end of June for October admission. Your reply is expected within seven calendar days.
