Positions
Up to two candidates are accepted on a competitive basis each year for the Nurse Practitioner or Physician Assistant General Surgery Fellowship at Mayo Clinic's Campus in Rochester, Minnesota.
Admission requirements
Prerequisites
To be eligible to apply for the Nurse Practitioner or Physician Assistant General Surgery Fellowship, you must have:
- Graduated, or anticipated graduation prior to program start, from an accredited nurse practitioner or physician assistant program. Candidates who have not graduated at the time of application submission must meet all subsequent prerequisites prior to program start.
- Licensure exam not required to be completed prior to submission of application but admission is contingent on passing licensure exam at least 90 days prior to planned start date
- National board certification
- Advanced Cardiovascular Life Support (ACLS) Basic Life Support (BLS) certification
Previous experience in a surgical practice is encouraged but not required.
Drug Enforcement Administration (DEA) registration is required upon acceptance into the program; expenses related to DEA registration are paid by Mayo Clinic School of Health Sciences..
Applicants are selected based on grades and work experiences, as well as voluntary experiences, leadership qualities, and representation of personal qualities and skills in a personal letter.

Non-U.S. citizen applicants
Admission to Mayo Clinic School of Health Sciences is open to U.S. workers in the four categories below. Therefore, visa sponsorship is not available.

Applicants whose primary language is not English must submit results from the Test of English as a Foreign Language Internet-based test (TOEFL iBT). Scores from the speaking portion of the exam are given particular consideration in admission decisions. Learn more about the exam and register online at the Educational Testing Service.
The Mayo Clinic School of Health Sciences code for the TOEFL iBT is 5784.
Educational transcripts from schools outside the U.S. must be translated (if they are not already in English) and evaluated for U.S. equivalence by an accredited credential evaluation service company prior to submission. Please refer to the National Association of Credential Evaluation Services for a list of qualified companies. The applicant pays for the examination and credential-evaluation services.

- Complete each section of the application and submit
- After submission, view the required Supplemental Items and Documents
- Upload each required item in the Supplemental Items section
- Complete the Recommendation Request section
Additional required items
- CV/resume
- Two letters of recommendation are required and must be completed in the Recommendation Request section. Each recommender will receive an email with a link to upload a letter and complete a rating form. At least one recommendation must be from a school professor or faculty member. At least one recommendation must be from a preceptor, supervisor, or colleague.
- Personal statement is to include the following and then be uploaded in the Supplemental Items section (maximum of 1200 words).
- Briefly summarize your past work experience.
- What influences your interests in a general surgery fellowship?
- How will this fellowship further develop your career goals in general surgery?
- Describe your short-term and long-term career goals after completion of the fellowship.
- Transcripts (official or unofficial) are required for all colleges/universities listed on the application.
- Unofficial college transcripts may be uploaded in the Supplemental Items for application purposes.
- If accepted, official transcripts will need to be emailed or mailed directly to the program.

Interview process
The selection committee carefully reviews each application. Individuals considered for an appointment will be initially interviewed by phone. Based on the phone interview, the candidate may be invited to Rochester, Minnesota, for an in-person interview.
Acceptance
Acceptance letters are mailed no later than July 31. Your reply is appreciated within seven days.
