Application Process

Positions

One candidate is accepted each academic year for the Nurse Practitioner or Physician Assistant Palliative Medicine Fellowship at Mayo Clinic's campus in Jacksonville, Florida.

Admission requirements

Prerequisites

To be eligible to apply for the Nurse Practitioner or Physician Assistant Palliative Medicine Fellowship, you must meet the following requirements upon acceptance into the program:

  • Graduated from an accredited nurse practitioner, family nurse practitioner, adult gerontology acute care, or adult gerontology primary care specialty, or physician assistant program, with a minimum of a master's degree
  • Hold a current license as master’s or doctorate-prepared nurse practitioner or physician assistant in the state of Florida or be eligible to apply for FL licensure at the time of offer acceptance, if applicable
  • Basic Life Support (BLS) certification*
  • Drug Enforcement Administration (DEA) registration*

*You do not need proof of DEA licensure or BLS certification at the time of application but must provide documentation of completion prior to fellowship start date.

Candidates are selected based on grades and work experience, as well as volunteer experience, leadership qualities, and representation of personal qualities and skills.

Admission policies

Non-U.S. citizen applicants

Admission to Mayo Clinic School of Health Sciences is open to U.S. workers in the four categories below. Therefore, visa sponsorship is not available.

  1. U.S. citizens
  2. U.S. nationals
  3. Lawful permanent residents
  4. Asylees and refugees

Applicants whose primary language is not English must submit results from the Test of English as a Foreign Language Internet-based test (TOEFL iBT). Scores from the speaking portion of the exam are given particular consideration in admission decisions. Learn more about the exam and register online at the Educational Testing Service.

The Mayo Clinic School of Health Sciences code for the TOEFL iBT is 5784.

Educational transcripts from schools outside the U.S. must be translated (if they are not already in English) and evaluated for U.S. equivalence by an accredited credential evaluation service company prior to submission. Please refer to the National Association of Credential Evaluation Services for a list of qualified companies. The applicant pays for the examination and credential-evaluation services.

Application timeline

The program begins in October 2026. Your application and all supporting documentation must be received by April 17, 2026.

Application instructions

Complete the following steps to apply:

  1. Create an account to begin the online Application for Admission.
    • Select - Mayo Clinic School of Health Sciences
    • Select - Nurse Practitioner/Physician Assistant Hospice and Palliative Medicine
  2. Complete each section of the application and submit.
  3. After submission, view the required Supplemental Items and Documents
    • Upload each required item in the Supplemental Items section
    • Complete the Recommendation Request section

Additional required items

  • Three letters of recommendation are required and must be completed in the Recommendation Request section. Each recommender will receive an email with a link to complete a rating form and upload a letter. Suggested recommendation types:
    • Academic (professor or faculty)
    • Professional (preceptor, supervisor, co-worker)
  • Personal Statement (one-page maximum) to be uploaded in the Supplemental Items section and address the following:
    • Why are you interested in a palliative care fellowship?
    • How do you plan to incorporate your fellowship training into future career goals?
  • Curriculum vitae
  • Unofficial college transcripts may be uploaded in the Supplemental Items area for application purposes. If admitted, official copies of all transcripts will be required prior to the start of the program. They must be sent directly from the institution by email to mack.rachel1@mayo.edu

Interview process

The selection committee carefully reviews each application. Individuals considered for an appointment are offered virtual interviews.

Acceptance

Appointment letters are mailed approximately June 5, 2026. Your reply is expected within seven days.
