The third year in fellowship at Mayo Clinic is funded by the division, and fellows do not have to seek grant funds for that year. To the best of my knowledge, this aspect is unique to Mayo Clinic. The three years of training allow for additional clinical, elective, and research time to prepare for one's eventual career.

One of the most unique features of the fellowship is the fact that despite it being a three-year fellowship, there is still an opportunity for equal distribution of that time between research and clinical training, so graduating fellows will not feel like they have been 'out of the clinical world' prior to entering into their own practices.
