Application Process
Positions
Five positions are available on a competitive basis each year in the Anesthesiology Critical Care Medicine Fellowship at Mayo Clinic's campus in Rochester, Minnesota.
Qualifications
You must be board-eligible or certified in anesthesiology to qualify for a fellowship position.

How to apply
All ACCM fellowships participate in the third-party application service SF Match, and all applications must be sent through their application portal. Applications and supporting documentation to our fellowship are due by Jan. 12, 2025, for the 2026-2027 academic year.
Applications must include the following:
- Common Application Service (CAS) application
- Personal info form
- Required supplemental form
- Personal statement
- Three reference letters*
*One letter must be from a training Program Director or Chair. We strongly encourage all applicants to have their Program Director and/or Chair complete the Core Program Director Standardized Letter of Evaluation form for their reference letter. Other reference letters should be from objective physicians (with one preferably from an intensivist) who have direct personal knowledge of your skills and ethics.

April 1, 2025 | Exception-to-the-match opens and exception offers are sent out to qualifying applications. |

May 27, 2025 | Match results are published. |

No obligation second look
Participating in numerous fellowship interviews can be logistically tricky and expensive. To help, we conduct all our interviews virtually. However, some applicants need to see the campuses and cities where they may train before deciding on a rank list. For them, we host a no-obligation visit. This visit is held after we have finalized and submitted our rank list, in other words, we cannot change our rank list based on who comes and how they conduct themselves during the visit.
Applicants, however, can change how they rank us. That’s the point: we want to provide an opportunity to see how great it is to train at Mayo so applicants can make an informed and confident decision without the usual pressure and stress from in-person visits. This process also ensures that everyone is evaluated equitably and those who cannot visit for logistical or financial reasons are not penalized.
Mayo will pay for one hotel night for those who can visit, but the applicants are responsible for transportation to/from Rochester. For further details on our no-obligation visit, please refer to the MCSGME general invitation letter.
Exceptions to SF Match
Positions may be offered early before final rank lists are due as an exception-to-the-match position. This process is outlined in the match rules and regulations set forth by the Society of Critical Care Anesthesiologists (SOCCA). Email us if you have questions or need clarification on the process to be offered a position as an exception to the SF Match. You can also review the match exception policies put out by the SOCCA Program Director’s Advisory Council (PDAC).
Dual applicants
Mayo Clinic not only provides the best training in anesthesiology critical care medicine but also the best training in adult cardiovascular anesthesia. Please refer to the Adult Cardiothoracic Anesthesiology Fellowship page if you are interested in ACA training. While we do not reserve a set number of dedicated spots for dual applicants each year, we do accept dual applicants on a case-by-case basis and are happy to explore that possibility with you and our sister ACA program.
Emergency medicine applicants
We do not offer training pathways for emergency medicine-trained applicants. However, Mayo Clinic’s outstanding Critical Care Internal Medicine Fellowship accepts EM-trained applicants through the American Board of Internal Medicine. We are integrated with them in much of our didactics and clinical experience and together provide broad-based training in all aspects of critical care.
