Application Process


There is one position available in Headache Fellowship Program at Mayo Clinic Arizona each year on a competitive basis.

How to Apply

Mayo Clinic School of Graduate Medical Education does not process printed applications to this program. To apply, you need to use the third-party application service, Apply Yourself.

In addition to the standard application materials required by Mayo Clinic School of Graduate Medical Education, this program requires the following additional supporting materials to be submitted with your online application:

  • A letter of recommendation from your residency program director or the department chair

You must submit a completed application form and all required documentation no later than January 1 of the academic year in which you wish to begin the fellowship (approximately 6  months in advance). Earlier applications are encouraged. The academic year begins in late June or early July.  

Should you be considered for an appointment, you will be invited to visit Mayo Clinic in Arizona for an interview with the program director and selected faculty.
