Pre-registration required documents
Your MCSGME onboarding package email sent via MedHub, our residency management system, has several onboarding requirements that you need to complete prior to orientation registration and program start date. See the Onboarding Document Checklist included in the onboarding package for who to contact with questions of a specific requirement(s).
Contact Provider Enrollment by May 1
- If you do not have a U.S. social security number, you cannot apply for an NPI number.
- Complete the NPI instructions once you have your social security number.
- If you have a National Provider Identifier (NPI) number, email it to Rochester Provider Enrollment.
- If you do not know your NPI number, you can look it up by first and last name in the NPI registry.
- If you do not have an NPI number but do have a social security number, apply for an NPI number using the NPI, CMS Identity, and Access instructions. Once you receive your assigned NPI number, email it Rochester Provider Enrollment.
- Complete the medical or dental licensure process as soon as possible.
Complete health review requirements
You'll receive an email regarding this process from Occupational Health Services (OHS) within 60 days of orientation or start date.
Complete your appointment with Human Resources
Schedule this appointment as soon as possible.
Inform us about special needs for orientation
If you have special needs, such as a sign language interpreter, lactation room, wheelchair access, or dietary needs, contact us.
