Pre-registration required documents
Your MCSGME onboarding package, sent through MedHub, our residency management system, contains several requirements that must be completed before you can register for orientation or begin your program. Please refer to the Onboarding Document Checklist included in the package for details on whom to contact regarding specific requirements.
Contact Provider Enrollment by May 1
- If you do not have a U.S. social security number, you cannot apply for an NPI number.
- Complete the NPI instructions once you have your social security number.
- For international medical graduates, this may occur after you’ve started your program.
- If you have a National Provider Identifier (NPI) number, email it to Rochester Provider Enrollment.
- If you do not know your NPI number, you can look it up by first and last name in the NPI registry.
- If you do not have an NPI number but do have a social security number, apply for an NPI number using the NPI, CMS Identity, and Access instructions. Once you receive your assigned NPI number, email it Rochester Provider Enrollment.
- Complete the medical or dental licensure process as soon as possible.
Complete health review requirements
You'll receive an email regarding this process from Occupational Health Services (OHS) within 60 days of orientation or start date.
Complete your appointment with Human Resources
Schedule this appointment as soon as possible. This appointment must be prior to your first date at Mayo Clinic.
Inform us about special needs for orientation
If you have special needs, such as a sign language interpreter, lactation room, wheelchair access, or dietary needs, contact us.
