Human Resources Work Authorization Requirements
Baseline background check
A baseline background check is required. It will be performed by Certiphi Screening, Inc., and can take several days to complete. International trainees can expect longer completion times.
You will receive an email from applicationstation@certiphi.com with instructions to complete your background check within 90 days of your orientation or start date. Be sure to start this process immediately upon receiving the email from Certiphi. Reminder emails will be sent daily until you complete the process. Ensure you have completed this at least 45 days prior to your orientation date.
Your appointment at Mayo Clinic School of Graduate Medical Education (MCSGME) is contingent upon successful completion of the background check. If your background check is not complete prior to your orientation date, you will not be able to start and your training will be delayed. If you have questions related to your baseline background check, please email Human Resources at HRReception@mayo.edu.
NOTE: Mayo Clinic must complete a Wisconsin Caregiver Background check for trainees completing a training in Wisconsin. Instructions will be provided if this is needed.
Human Resources appointment
I-9 Appointment
A virtual appointment with Human Resources is required to complete Section 2 of the Form I-9. Be sure to complete this 15-minute virtual appointment prior to your orientation date.
Using a secure or encrypted email, send your I-9 documents to HRReception@mayo.edu in advance of the appointment. You will also need to show these documents during your virtual appointment.

Self-schedule your virtual I-9 HR appointment.
Do not schedule the appointment after your start date.
Minnesota Background Study Appointment
An in-person appointment with Human Resources is required to complete your Minnesota Background Study requirements. Be sure to complete this 30-minute in-person appointment prior to your orientation date.
Once you self-schedule your appointment you will receive a confirmation email that contains a link to reschedule the appointment if necessary. Be sure to schedule your HR appointment ASAP to ensure an appointment time that is convenient for you and coordinates well with your onboarding schedule.
If you visit Rochester prior to your orientation date, you are expected to complete the appointment during this visit. If this is not possible, you can schedule your Human Resources appointment before or on your orientation date. Do not schedule the appointment after your start date.
Self-schedule your in-person HR appointment.
You will need to bring the identification document you used to initiate your Minnesota Background Study to your in-person appointment.

Minnesota background study
You must initiate the online portion of the background study prior to your appointment with Human Resources. This background study is different than the Baseline Background Check and is required by the State of Minnesota for individuals who will be working in Minnesota and have direct patient contact.
The following information is needed to initiate the study:
- Provider name and license number: Mayo Clinic Hospital – Rochester, 428
- Provider number: MAY33V
Be sure to enter your name exactly as it appears on the identification document that you'll present during your in-person Human Resources appointment.
The fingerprinting and photograph requirements for the Minnesota background study will be completed during your in-person appointment with Human Resources.
Note: The licensure background study and fingerprints do not meet this requirement. This is a separate process that must be completed per Minnesota Background Study requirements. Certain licensed individuals may be excluded from this requirement. Human Resources will make this determination at your human resources appointment.
Your appointment with Mayo Clinic School of Graduate Medical Education is contingent upon the successful completion of the Minnesota Background Study.

Employment Eligibility Verification (I-9 Form)
Form I-9 is used to verify the identity and employment authorization of individuals working in the United States. There are three steps to this process: Completing Section 1 using an online process through Certiphi Screening, Inc., emailing your I-9 documents to HR using a secure email, and completing Section 2 during a virtual appointment with a Mayo Clinic HR Representative.
Wait to start this process until you have received authorization to work in the United States and have a United States address.
Step 1: Completing Section 1 of Form I-9:
- Receive and read through the instructions in the email you receive from i9Success@certiphi.com, subject line: i9Success I-9 Request.
- Click on the link in the email you receive
- Click “Login” using your credentials that were previously created for your National Background Check (Certiphi Screening, Inc.), or click “Sign-Up” to create a new account. Once you are signed in, you will be taken directly to Section 1 of the I-9 form.
- Complete the form in its entirety; enter all personal information and click “Continue.”
- Read through the Confirmation screen; it contains important information including identification information required to complete Section 2 of the I-9 form.

Step 2: Email your I-9 documents to HR using secure email.
- You will receive a secure email from HRReception@mayo.edu.
- The subject line will say: [MayoSecure] Secure Email from Mayo Clinic: HR Information Request.
- Respond to this secure email, prior to your virtual appointment with Mayo Clinic HR, and attach your I-9 documents including a copy of your original U.S. Social Security (SS) card, for payroll purposes.
Step 3: Completing Section 2 of Form I-9:
- Respond to this secure email, prior to your virtual appointment with Mayo Clinic HR, and attach your I-9 documents including a copy of your original U.S. Social Security (SS) card, for payroll purposes.
- Schedule a virtual appointment with Mayo Clinic HR to complete Section 2 of the I-9 form.
- During your virtual I-9 appointment.
- Show your I-9 documents to the Mayo Clinic HR representative.

Social Security card
Internal Revenue Service regulations require that all payroll documentation, including the name on your paycheck, must be exactly as it appears on your Social Security card. Be sure to email an original copy of your Social Security card to HRReception@mayo.edu in advance of your virtual I-9 appointment using the secure email.
International trainees without a Social Security number (SSN): You will need to call the local Social Security Administration (SSA) office and request an appointment to apply for a number in person. SSA recommends waiting 10 days after entry into the U.S. to apply for an SSN to allow their electronic systems to be updated with your immigration status.
Upon receipt of your original card with the number, you will need to present it to Human Resources.
