Fellow Process
Allopathic
Full Medical Doctor licensure with the Florida Board of Medicine is required before beginning your fellowship program. The process requires three to six months to complete. Complete the Application for Medical Doctor. Appropriate information to be used on the application:
- Financial Responsibility – Response #1
- NICA Exempt (EPC will provide documentation to the board directly)
- In-training program will offer a reduced application fee
- Trainees do not dispense medication – Answer is No
Submit the completed application online via the MQA portal. Supporting documentation and payment are submitted directly to the Florida Board of Medicine. Mayo Clinic will provide reimbursement for all expenses associated with obtaining your medical license except for a third party you hire to complete the process on your behalf (example: FCVS) once you begin your training program. Your education program coordinator will assist you with the reimbursement process during Orientation. Retain your receipt for the submission.
Osteopathic
Full licensure with the Board of Osteopathic Medicine is required before beginning your fellowship program. This process requires three to six months to complete. Complete the Application for Osteopathic Medicine Full Licensure.
Submit the completed application via the MQA portal. Supporting documentation and payment are submitted directly to the Florida Board of Osteopathic Medicine. Mayo Clinic will provide reimbursement for all expenses associated with obtaining your medical license except for a third party you hire to complete the process on your behalf (example: FCVS) once you begin your training program. Your education program coordinator will assist you with the reimbursement process during Orientation. Retain your receipt for the submission.
Intern Process
Allopathic
Registration with the Florida Board of Medicine is required before beginning your residency program. This process requires two to three months to complete. Complete the Application for Registration as an Unlicensed Allopathic Physician. Appropriate information to be used on the application:
- Employment date: June 24, 2024
- University/hospital: Mayo Clinic
- Program specialty: Include your categorical program name
- Director of Medical Education: Margaret Johnson, M.D.
- Program address: 4500 San Pablo Road, Jacksonville, Florida 32224
- Clinical sites: Vickers Landing, Sulzbacher, St. Vincent’s, and Mission House
- Name/phone of administrator: Jamila Donnal, 904-953-2000

Exam deadlines
Mayo Clinic School of Graduate Medical Education (MCSGME) requires that your final licensing exams be taken as follows:
